Camden's Promise Charter School and Camden's own, Project H.O.P.E, have partnered up to provide COVID-19 Vaccinations for children and teens 12 years and older and their families in our school community. The date of this particular event will be July 20, 2021 at 1pm and will take place at Camden Academy Charter High School located at 250 Federal St, Camden, NJ 08103.
While the vaccine is not mandatory, the CDC recommends vaccination for everyone 12 years and older to help protect against COVID-19.
If you would be interested in obtaining vaccination for your child or family during this event, please fill out the survey with interest and the number of household members that would be interested in participating in this event. Please also fill out the attached EUA and consent form if you would like to receive the vaccine.
**Keep in mind that this is a 2-dose vaccination series in which the 2nd vaccination will be scheduled approximately 21 days after the first vaccination.**

Camden's Promise Charter School y el propio, Proyecto H.O.P.E, de Camden se han asociado para proporcionar vacunación contra el COVID-19 para niños y adolescentes de 12 años en adelante y sus familias en nuestra comunidad escolar. La fecha de este evento será el 20 de julio de 2021 a la 1 p.m. y se llevará a cabo en Camden Academy Charter High School ubicada en 250 Federal St, Camden, NJ 08103.
Si bien la vacuna no es obligatoria, los CDC recomiendan la vacunación para todas las personas mayores de 12 años para ayudar a protegerse contra el COVID-19.
Si estaría interesado en obtener la vacuna para su hijo o familia durante este evento, complete la encuesta con interés y la cantidad de miembros del hogar que estarían interesados en participar en este evento. También complete el formulario de consentimiento y EUA adjunto si desea recibir la vacuna.
