Required Immunizations for all Students Entering 6th Grade
According to State Regulations, all immunizations must be current
for students to enroll or continue in school.
Your child must have a
MENACTRA AND Tdap IMMUNIZATION
BEFORE THE START OF THE SCHOOL YEAR WHICH BEGINS
SEPTEMBER 8, 2010.
IF WE DO NOT RECEIVE DOCUMENTATION THAT YOUR CHILD
RECEIVED THE ABOVE IMMUNIZATIONS BY
SEPTEMBER 8, 2010,
YOUR CHILD WILL BE EXCLUDED FROM SCHOOL UNTIL THE
REQUIREMENTS HAVE BEEN MET.
